MISSION STATEMENT
It is the mission of Louisville Urology to treat all patients with compassion
and respect. We strive to provide the best and most comprehensive
urologic care available, and to provide this care with integrity and pride. Our
goal is to maintain ethical standards and values in compliance with the
laws and regulations that govern the Health Care Industry.
FINANCIAL POLICY - Louisville Urology participates
with most insurance plans, and will file all insurance claims for the
convenience of our patients, but it is ultimately the responsibility of
the patient to ensure that our doctors are paid for their services. We
will enlist the aide of the patient or subscriber to help get problem
claims resolved whenever it is necessary.
Co-pays: We accept payment in the form of cash, checks,
or credit/debit cards. Payment
of co-pays is required at the time of service. The appointment will
be rescheduled if a patient is unable to make the co-pay prior to being
seen.
Insurance Assignment: We will file all insurance claims,
either electronically or on paper. We do accept Medicare assignment.
Medicare pays 80%, and patients will be asked to pay the other 20% at the
time of service if there is no secondary or supplemental insurance to file.
No Insurance: We are happy to treat patients who do not
have insurance, however payment in full at the time of service is required. An
estimated amount will be quoted at the time the appointment is made. Patients
who fail to bring the required payment will be asked to reschedule.
Payment Agreements: If the co-insurance amount presents
a financial hardship, patients should call the office, and speak with the
Office Manager to set up a mutually agreeable payment plan.
Disability Insurance Forms: This is a $10.00 fee
for all forms that need to be completed.

If you have insurance that is not listed above, or if you have any questions
regarding your insurance, please call our office at (502) 375-0009 and
ask for the billing office. We will be happy to verify our participation
status, and assist you in any way we can.
Referrals: Patients must obtain a referral from their
primary care physician if they belong to an insurance plan that requires
one. The referral must
be brought to the office at the time of the appointment. Following
